How will we know if it is helping? When would we review the plan? What should I track at home?
Symptom management questions
Use this section to talk about daily symptoms. Do not start, stop, or change medicine from this sheet.
Which symptoms can we try to reduce first? What can I do on heavy bleeding days? What can I do on high pain days? When should bleeding be treated as urgent? When should pain be treated as urgent? Should I be checked for anemia? Should I ask about iron or ferritin testing? What symptoms should make me call sooner? What should I track until follow up?
Hormonal option questions
Use this section if hormonal care comes up.
What type of hormonal option are we discussing? Is it meant to reduce bleeding, pain, or both? How might it change my cycle? How soon would we review it? What side effects should I track? What side effects should I report right away? Could it affect fertility goals now or later? Could it interact with my other medicines or supplements? What should I do if bleeding changes? What happens if it does not help enough? How long would we try this plan for my case?
Procedure questions
Use this section if a procedure is discussed.
What procedure are we discussing? What symptom is it meant to help? What symptoms might remain? What imaging is needed first? What are the main risks? What recovery symptoms should I expect? How could it affect future pregnancy? Would I need time off work, school, or care tasks? What follow up would I need? What would make this a poor fit for me? Are there other options to compare first? Should I see a specialist before deciding?
Hysterectomy discussion questions
Use this section only if hysterectomy comes up. Ask for plain terms.
Why is hysterectomy being discussed? Which symptoms might it help? Which symptoms might not change? What organs or tissue would be removed? Would ovaries be discussed too? What are the main risks? What recovery time should I plan for? What help would I need at home? How would this affect fertility? How would this affect bleeding? What follow up would I need? Are there other options for my case? Is a second opinion reasonable?
This list does not recommend hysterectomy. It helps you ask what it would mean.
Fertility and pregnancy goal questions
Use this section only if it matters to you.
How could each option affect pregnancy now? How could each option affect pregnancy later? Could adenomyosis affect fertility or pregnancy planning? Should I talk with a fertility specialist? What timing issues should I know about? What symptoms should I track while deciding? What information would help the next visit?
You do not have to share fertility goals if they do not fit your visit.
Anemia and bleeding questions
Heavy bleeding can affect daily life. Ask what testing fits your case.
Do I need a complete blood count? Do I need iron or ferritin checked? Could bleeding be causing fatigue or dizziness? What level of bleeding is urgent for me? How should I track clots or flooding? Should I bring a bleeding log to follow up? What should I do if bleeding gets worse before follow up?
Use the heavy period visit prep checklist or abnormal bleeding diary if bleeding is your main concern.
Side effect and follow up questions
Ask how the plan will be checked.

What to track while deciding
Track enough to make the next visit useful.

Private notes
Adenomyosis decision notes can include bleeding, pain, sex pain, fertility, imaging, medicine, surgery, and insurance concerns. Share the smallest useful record.
Ask:
What data do you need for this decision? Is a one page symptom summary enough? Do you need full cycle history? Will this go in my medical record? Can I leave out private notes? Can I bring a short summary instead of a full export?
