Adenomyosis Bleeding and Clot Log

A simple log for adenomyosis bleeding patterns, clots, flooding, product changes, pain, pressure, and anemia symptoms before care.

Heavy bleeding can be hard to explain from memory.

This log gives you one place to track flow, clots, flooding, pain, pressure, and symptoms that may fit low iron. It cannot diagnose adenomyosis.

Do not try to identify a clot, tissue, or a cause by sight. Write what you saw in plain words. Ask a clinician if you are worried.

Seek urgent or emergency care for severe or heavy bleeding, faintness, chest pain, pregnancy concern, severe pain, fever, inability to pee, or any time waiting feels unsafe.

Daily bleeding log

Use one row per day. Plain notes are enough.

If you use a cup or disc, add how often you empty it. Add the amount only if you know it.

Words like "quarter size" or "larger than a quarter" are enough. You do not need photos unless you choose to take them.

Do not force a pattern. Write what happened.

Symptoms after heavy days

Mark what happens during or after heavy bleeding.

I feel very tired. I feel dizzy or lightheaded. I feel weak. I get short of breath. My heart feels like it is racing. I have a headache. I look pale compared with my usual. I missed work, school, sleep, care tasks, or plans.

Ask your clinician whether anemia testing fits your symptoms.

What changed this cycle
