Fibroid Bleeding and Clot Log
A simple log for heavy bleeding, clots, flooding, product changes, pain, and fatigue when fibroids may be part of the concern.
Seeing clots in the toilet can be upsetting. It can also be hard to describe later.
Do not try to tell what something is by sight. You cannot confirm a clot, tissue, or fibroid from appearance alone. Track the pattern and call a clinician if you are worried.
If bleeding feels severe or unsafe, seek urgent or emergency care. Do not wait to finish this log.

Use one row per day. Plain notes work well.

If you use a cup or disc, add the amount if you know it. If you do not know, write how often you emptied it.

Do not use this table to identify what you saw. Use it to show your clinician the pattern.

Words like "quarter size" or "larger than a quarter" are enough. You do not need photos unless you choose to take them.
Product change notes
This helps show how fast bleeding is happening.

Add night changes too. Waking up to change protection is useful to report.
Symptoms that can go with heavy bleeding
Mark what happens during or after heavy days.
Fatigue. Dizziness. Weakness. Shortness of breath. Headache. Fast heartbeat. Pelvic pressure. Cramps or belly pain. Back pain. Missed work, school, sleep, or plans.
Ask your clinician whether anemia testing fits your symptoms.

Does this count as heavy menstrual bleeding? Could fibroids fit this pattern? What else could cause these clots or bleeding? Do I need a pregnancy test today? Do I need a blood count or iron test? Do I need an ultrasound? What symptoms mean I should seek urgent care? What should I track next cycle?
