Adenomyosis vs Endometriosis vs Fibroids Checklist

A plain visit-prep checklist for sorting symptom patterns that may overlap across adenomyosis, endometriosis, and fibroids. It is not a self-diagnosis tool.

Use this before a visit if you are trying to explain symptoms.

This is not a quiz. It cannot tell you if you have adenomyosis, endometriosis, or fibroids. It helps you bring clearer notes to care.

Start with what changed

Short notes are enough.

Question Your notes What symptom made you look for help? When did it start? What was normal for you before? What is different now? What symptom affects life most? What do you want checked first?

If symptoms are severe, new, or unsafe, do not wait to finish this sheet. Seek urgent or emergency care.

Pattern checklist

Check what fits. Leave the rest blank.

Symptom pattern Check Notes Periods are heavier than before Periods last longer than before Clots are new or more common Bleeding happens between periods Pain starts before bleeding Pain is worst during bleeding Pain lasts after bleeding ends Pain happens outside periods Pain is deep in the pelvis Pain is one sided Pelvic pressure or heaviness Lower belly fullness Frequent urination Constipation or rectal pressure Pain with bowel movements near periods Pain when peeing near periods Pain with sex, if relevant Fatigue, dizziness, or shortness of breath Trouble getting pregnant, if relevant

Do not use check marks to self diagnose. Use them to show a clinician what repeats.

Side by side notes

Use this table to sort what you want to ask about. It does not tell you which condition you have.

Pattern to ask about Adenomyosis question Endometriosis question Fibroids question Heavy or long bleeding Could this fit adenomyosis or another bleeding cause? Could endometriosis also affect my cycle or pain pattern? Could fibroids explain heavy bleeding or clots? Pain timing Could pain tied to heavy bleeding fit adenomyosis? Could pain outside bleeding, sex pain, bowel pain, or bladder pain fit endometriosis? Could fibroids cause pain or pressure in my case? Pelvic pressure or belly fullness Could an enlarged or tender uterus be part of this? Could pain or inflammation feel like pressure? Could fibroid size or location cause pressure? Bowel or bladder symptoms Should these be checked along with pelvic symptoms? Could period timed bowel or bladder pain matter? Could pressure on the bladder or bowel matter? Imaging questions What can ultrasound or MRI show or miss? Would imaging or referral help check for endometriosis related findings? Did imaging show fibroids, size, or location? Fertility questions Could this affect pregnancy goals or planning? Could endometriosis affect fertility goals? Could fibroid location affect fertility or pregnancy goals?

Use the adenomyosis ultrasound and MRI question list if imaging is part of the next visit.

Bleeding notes

Heavy bleeding can point to several possible causes. It can also cause anemia.

Date Period day Flow Products used Clots Leaks Pain 0 to 10 : : light, medium, heavy, very heavy none, small, quarter size, larger

Ask:

Does my bleeding count as heavy menstrual bleeding? Should I have a blood count? Should I have iron or ferritin checked? Could fibroids, adenomyosis, or another cause fit? Do I need an ultrasound or other testing?

Pain timing notes

Timing can help your clinician choose next steps.

Timing Your notes Pain before the period Pain during the period Pain after bleeding stops Pain near ovulation Pain all month Pain with bowel movements Pain with urination Pain with sex, if relevant

Ask:

What does the timing suggest? What else could cause this pain? Does this pattern need imaging? Does this pattern need referral? What symptoms should make me call sooner?

Pressure notes

Pressure can be hard to describe. Write plain details.

Pressure symptom Your notes Pelvic heaviness Lower belly fullness Pants feel tighter at the lower belly Frequent urination Trouble emptying bladder Constipation Rectal pressure Symptoms worse during period Symptoms happen all month

Ask whether pressure could fit fibroids, adenomyosis, bowel issues, bladder issues, or another cause.

Imaging and test history

Fill this out if you have had tests before.

Test or visit Date Result words you remember Follow up Pelvic exam Ultrasound MRI Blood count or iron test Other test

Bring reports if you have them. If you do not, bring what you remember.

Questions to bring

What conditions could fit this pattern? What conditions are less likely? What do you need to check first? Do I need labs for anemia or another cause? Do I need imaging? What can imaging show? What can imaging miss? When would referral make sense? What should I track before the next visit? What symptoms mean I should call right away?

Short visit script

"I am not trying to diagnose myself. I am trying to show the pattern. My main symptom is . It started . It is worst . It affects my life by . What should we check next?"

If the visit is short, start with daily impact. Missed work, school, sleep, sex, care tasks, and plans all matter.

Privacy note

These notes can include bleeding, sex pain, bowel symptoms, bladder symptoms, fertility, and pregnancy details.

You can bring a short summary instead of a full app export. Ask what details are needed for the visit. Ask what will go in your medical record.

Floriva can help you keep local notes and make a short summary. Paper works too. Use the format that fits your visit and your privacy needs.

Use the adenomyosis data privacy checklist if your notes include sex pain, fertility, imaging, or full app exports.
