Itching, burning, soreness, and discharge changes can feel obvious. They are not proof of yeast.
Other problems can feel similar. This log helps you bring clear notes to care. It does not diagnose you. It does not tell you what medicine to use.

Check what fits.
Symptoms started after a period. Symptoms started before a period. Discharge changed in amount. Discharge changed in texture. Odor changed. Itching is the main problem. Burning is the main problem. Peeing hurts. Sex hurts, if you choose to share. I used a vaginal product, wipe, soap, or scent. I took antibiotics recently, if relevant. I have diabetes or blood sugar concerns, if relevant. I am pregnant or may be pregnant, if relevant.
These notes do not prove the cause. They help your clinician decide what to ask next.

Questions to ask
Pick the questions you want to bring.
Could this be yeast, or could it be another cause? Do I need a test before treatment? Should we test vaginal pH, discharge, or a swab? Could BV, trichomoniasis, cervicitis, irritation, or another issue fit? Could recent antibiotics matter? Could diabetes or pregnancy context matter for care? What symptoms mean I should call right away? What should I track if symptoms come back?
For discharge notes, use the vaginal discharge color and odor checklist. If timing matters, use the discharge before period tracker. If odor after bleeding is the main issue, use the vaginal odor after period log.
If itching or burning is the main symptom, use the vulvar itching symptom log or the vulvar burning symptom notes.
