Recurrent Yeast Infection Appointment Prep
A visit prep sheet for repeated yeast-like symptoms, testing history, symptom return, medicine already used, health context, and specialist questions.
Repeated itching, burning, soreness, or discharge changes can wear you down. It can also be easy to guess wrong.
Use this sheet before a visit. It helps you show what came back, what was tested, and what you already used. It does not diagnose you or tell you what to take.
Visit goal
Pick the main thing you want from the visit.
Goal Check I want to know if this is yeast or something else. I want testing before more treatment. I want to review why symptoms keep coming back. I want to review medicine I already used. I want to ask about a specialist. I want to know what to track next.
My main goal:
Your note
Symptom return timeline
Write one row for each time symptoms came back.
Date symptoms started Main symptoms Discharge or odor change Test done? Medicine used Did symptoms return? yes / no / not sure yes / no / not sure
Do not worry if dates are rough. A month or week can still help.
Testing history
Bring test names or portal screenshots if you have them. A short summary is fine.
Test or visit Date Result, if known Question Wet mount or microscope test Vaginal pH Yeast culture NAAT or swab panel STI testing, if done Diabetes or blood sugar test, if relevant Pregnancy test, if relevant
Questions to ask:
What tests have I already had? Did any test confirm yeast? If yeast was found, do we know the type? Should culture or other testing be done? Could BV, trichomoniasis, cervicitis, irritation, or another cause fit? Do my results change what we do next?
Medicine already used
List prescription and over the counter products. Do not use this page to choose a product or dose.
Medicine or product Start date How used Helped? Side effects or notes yes / no / not sure
Questions to ask:
Could any medicine I used affect test results? Did I use the right product for the confirmed cause? What should I do before using more medicine? What symptoms mean I should call instead of waiting?
Context to share if it fits
Only share what is needed for care and what you are comfortable sharing.
Context Your note Recent antibiotics Diabetes or blood sugar concerns Pregnancy or possible pregnancy Immune system condition or medicine, if relevant New soap, wipes, pads, lubricants, or products New partner or STI testing question, if relevant Symptoms after sex, if you choose to share Symptoms tied to period timing
CDC and ACOG both warn that symptoms can overlap. Your clinician can decide what context matters.
Specialist questions
Ask these if symptoms keep returning or the plan is unclear.
Should I see a gynecologist? Should I see a vulvovaginal specialist? Should infectious disease be involved? Should we confirm the cause before more treatment? Should we test for non albicans yeast? What should I track before the next visit? What should I avoid doing before testing? How will we know whether the plan worked?
One page appointment summary
Fill this out before the visit.
Summary item Your note Number of symptom returns I remember First return date Most recent return date Main symptom each time Tests done Test results I know Medicine already used Antibiotics, diabetes, or pregnancy context, if relevant Main specialist question What I want help with today
Build the rest of your notes
Use the yeast infection symptom log for daily symptoms. Use the BV symptom visit prep checklist if odor or discharge changes make you wonder about BV. Use the vaginal discharge vs cervical mucus checklist if you are not sure whether a change fits your usual cycle pattern.
For more discharge context, read discharge before period. For privacy, read the vaginal discharge data privacy checklist.
Privacy note
Repeat symptom notes can include sex, tests, pregnancy, and medicine. Keep the record where you control it.
Floriva can keep dates and symptom notes on your device. Paper works too. Bring only the summary if that gives your clinician enough to start.
