Use this log when odor shows up after a period. It helps you see the pattern before a visit.

This log cannot diagnose BV, yeast, STIs, or any other cause. It does not tell you how to treat odor.

Plain notes help. "Fishy smell on day 2 after period" is useful. "Bad" is okay if that is all you can write.

What else changed

Check what fits. Skip anything too private to write down.

Odor started after bleeding stopped. Odor started while spotting continued. Discharge amount changed. Discharge color changed. Discharge feels thin. Discharge feels thick. Itching started. Burning started. Peeing burns. I have pelvic pain. I have fever. I have sores or skin pain. I may be pregnant. This pattern repeats most cycles.

Do you need my full cycle history? Is this short log enough? Will these notes go in my chart? Can I leave out notes that do not affect care?
