UTI vs Yeast Symptom Notes

A no-diagnosis note sheet for urinary burning, urgency, pelvic pressure, itching, soreness, discharge changes, odor, timing, and clinician questions.

Use this sheet when pee symptoms and vaginal symptoms are hard to sort out.

This page is not a diagnosis. It will not tell you if you have a UTI, yeast infection, BV, STI, irritation, or another cause. It helps you write clear notes.

Overlapping urinary and vaginal symptoms can need testing. Discuss fever, back or side pain, nausea or vomiting, blood in urine, pregnancy concern, severe or worsening symptoms, or new intense pelvic pain with a clinician promptly.

Plain words help. "Burns at the end of peeing" is useful. So is "itching is worse than pee pain."

Symptom check

Check what fits.

Burning starts when urine touches the skin. Burning feels deep or inside. I need to pee often. I need to pee right away. Only a little urine comes out. My lower belly feels sore or heavy. My vulva or vagina itches. My vulva or vagina feels sore or swollen. Discharge amount changed. Discharge color or texture changed. Odor changed. I see blood in urine. Symptoms started near my period. Symptoms started after sex, if relevant. I used a new soap, wipe, scent, lubricant, or vaginal product. I took antibiotics recently, if relevant.

These notes do not prove the cause. They help you ask for the right check.

Questions to ask

Could this be a UTI, yeast, BV, STI, irritation, or something else? Do I need a urine test? Do I need a vaginal swab or exam? Could pregnancy change what should be checked? Could blood in urine need urgent care? What symptoms mean I should call right away? What should I track if this happens again?

For a UTI only note, use the UTI symptom tracker. If symptoms show up near bleeding, use the UTI during period symptom log. For discharge notes, use the vaginal discharge color and odor checklist. For outside burning or itching, use the vulvar burning symptom notes or vulvar itching symptom log.
