UTI Symptom Tracker, No Diagnosis

A no-diagnosis UTI symptom tracker for burning, urgency, frequency, lower belly pressure, urine changes, fever, back pain, and visit questions.

UTI symptoms can feel sharp and clear. They still do not prove the cause.

Use this sheet to track what you feel, when it started, and what changed. Bring the notes to care. Do not use this page to choose medicine.

What changed?

Check what fits.

Burning is the main problem. I feel like I need to pee right away. I pee often, but only a little comes out. I feel lower belly pressure. Urine looks cloudy. Urine smells different. I saw blood in urine. I have fever. I have back or side pain. I feel sick to my stomach. I vomited. I am pregnant or may be pregnant. Symptoms started near sex, if I choose to share. Symptoms started near my period, if relevant.

Timing can help your clinician ask better questions. It does not prove what caused the symptoms.

Questions to ask

Pick the questions you want to bring.

Do my symptoms fit a UTI, or could this be something else? Do I need a urine test? Do I need a urine culture? Should pregnancy change what we check? What symptoms mean I should call right away? Could this be bladder pain, vaginal irritation, or yeast instead? What should I track if this happens again?

For symptoms after sex, use the UTI after sex visit prep checklist. For repeat symptoms, use the recurrent UTI appointment prep checklist. If yeast is also on your mind, use UTI vs yeast symptom notes.
