Recurrent UTI Appointment Prep Checklist

A plain appointment prep sheet for repeated UTI symptoms, with symptom dates, urine test history, culture questions, pregnancy context, warning signs, and private notes.

Use this before a visit for UTIs or UTI like symptoms that keep coming back.

This checklist does not diagnose recurrent UTI. It does not choose treatment. It helps you bring dates, tests, and questions.

Discuss symptoms promptly with a clinician if you have fever, back or side pain, nausea, vomiting, blood in urine, pregnancy concern, severe symptoms, or symptoms that are getting worse.

Episode timeline

Write one row for each episode you remember.

Ask what each result means. Also ask what it does not rule out.

Questions to ask

Do my records fit recurrent UTI? Do my symptoms need urine testing today? Do I need a urine culture? Were past cultures positive? What bacteria were found, if any? Could another cause fit my symptoms? Should pregnancy change what we check? Do I need follow up with a specialist? What symptoms mean I should call sooner? What should I track before the next visit?

Context that may matter

Only share what is needed for care.

Sex timing can be one clue in the visit story. It does not mean sex caused your symptoms.

Warning signs to name early

Tell the clinician early if any of these fit.

Fever. Chills. Back pain. Side pain. Nausea. Vomiting. Blood in urine. Pregnancy or possible pregnancy. Symptoms that feel severe. Symptoms that are getting worse.

Mayo Clinic says back or side pain, high fever, chills, nausea, and vomiting can happen when the kidneys are affected.

Short visit script

Private notes

Repeated UTI notes can include sex timing, pregnancy worry, urine culture results, medicine, and location clues. Share the smallest useful record.

Ask:

Do you need my full app history? Is this summary enough? Will these notes go in my chart? Can I leave out private notes not tied to care?
