UTI After Sex Visit Prep Checklist

A plain visit prep sheet for urinary symptoms that show up near sex, with symptom timing, urine test questions, pregnancy context, warning signs, and private notes.

Use this before a visit for burning, urgency, pelvic pressure, or pee changes that show up near sex.

This checklist does not diagnose a UTI. It does not tell you what medicine to take. It helps you bring clear notes and better questions.

Discuss symptoms promptly with a clinician if you have fever, back or side pain, nausea, vomiting, blood in urine, pregnancy concern, severe symptoms, or symptoms that are getting worse.

One page visit sheet

Fill this out first.

Question Your note Main symptom Date symptoms started Burning when peeing Strong urge to pee Peeing often, with little urine Lower belly or pelvic pressure Cloudy, bloody, or strong smelling urine Fever, chills, back pain, or side pain Nausea or vomiting Pregnancy or possible pregnancy Sex timing, if you want to share it Main question for the clinician

Symptom timing notes

Write what happened. Keep it plain.

Date Symptom How strong was it? What was near that time? What did you do next? mild / medium / strong sex / period / other / not sure

Sex timing can be context a clinician may ask about. It does not mean sex caused your symptoms.

What changed

Check what fits. Leave the rest blank.

Peeing burns. I need to pee often. The urge does not go away. Only a little urine comes out. I have lower belly pressure. I have pelvic pain. Urine looks cloudy. Urine smells different. I see blood in urine. I have fever or chills. I have back or side pain. I have nausea or vomiting. I may be pregnant. Symptoms came back after a past UTI visit.

Questions to ask

Do my symptoms need a urine test? Do I need a urine culture? How should I collect the urine sample? Could this be something other than a UTI? Should pregnancy change what we check? What symptoms mean I should call sooner? What should I track if this happens again? Do you need my full history, or is this summary enough?

Do not use this checklist to start, stop, or choose antibiotics. Ask your clinician what care fits you.

Testing notes

Ask what each test can show.

Test topic Question to ask Urine dip or urine analysis What did it check? Urine culture Do we need this today? Pregnancy test Does this fit my visit? STI testing Could this fit my symptoms? Follow up test When should I ask again?

Mayo Clinic says a urine culture may follow urine analysis. Ask if that fits your symptoms and history.

Short visit script

"My main symptom is . It started . It showed up near sex on , if that matters. I also have . Can we talk about what to test?"

You can skip any detail that does not feel needed for care.

Private notes

UTI notes can include sex timing, pregnancy worry, test results, and location clues. Share the smallest useful record.

Ask:

Do you need my full app history? Is this one page summary enough? Will these notes go in my chart? Can I leave out private notes not tied to care?

Floriva can keep dates and notes on your device. Paper works too.

Use the UTI symptom tracker for daily notes. Use the UTI during period symptom log if symptoms overlap with bleeding days. Use the UTI vs yeast symptom notes if burning, discharge, or itch make the pattern unclear.
