Painful Bowel Movements During Period Notes

A no-diagnosis visit prep worksheet for painful bowel movements during period timing, bowel pain during period days, stool changes, pelvic pain, bleeding, and questions for care.

Pain with pooping can feel private. It can also be hard to explain fast.

Use this sheet to write what happened. Bring the pattern to care. Do not use this page to name the cause or pick treatment.

Prompt care check

Seek care promptly if any of these fit.

Severe abdominal or pelvic pain Pain is getting worse Fever Vomiting Blood in stool Black stool Rectal bleeding Blood in urine Signs of dehydration, such as very little pee or feeling very weak Pregnancy or possible pregnancy Faintness or dizziness Unexplained weight loss Symptoms feel unsafe

Do not wait for a perfect log if you feel very sick.

Quick visit note

Fill this in before you call or go in.

Question Your answer Date symptoms started Cycle day or period day Main pain location Worst bowel pain, 0 to 10 Worst pelvic pain, 0 to 10 Stool was loose, hard, or usual Diarrhea Constipation Gas or bloating Nausea or vomiting Fever Blood or black stool Rectal bleeding Blood in urine Pregnancy concern Main question for care

Plain words are enough. "It hurts to poop on day 1 and 2 of my period" is a useful note.

Three cycle bowel pain log

Use one row for each day pain shows up.

Date Cycle day Period day Bowel pain 0 to 10 Pelvic pain 0 to 10 Stool type Bleeding concern Notes : : loose / hard / usual yes / no

Track timing. Do not decide that your period, a food, IBS, or endometriosis caused it. The goal is a clear visit note.

What changed from your usual pattern?

Check what fits.

Pain started before bleeding. Pain started with bleeding. Pain is worst when I poop. Pain stays after I poop. I feel rectal pressure. I feel pelvic pressure. Stool is looser than usual. Stool is harder than usual. I have more gas or bloating. I have nausea. I vomited. I saw blood. Stool looked black. I also have bladder pain or blood in urine. This is new for me. This is a repeat pattern.

Use the period diarrhea symptom log, period constipation log, or period gas and bloating log if one symptom needs its own page.

Food, medicine, and context notes

Keep this short. Add only what you want to ask about.

Date Note type Short note Ask about it? Food yes / no Medicine yes / no Stress or sleep yes / no Sex timing yes / no Pregnancy concern yes / no Other yes / no

You do not need to write a full diary. A short note may be enough.

Questions to ask

Pick the questions that fit.

Could this pain be linked to my cycle, or could it be something else? Do my notes suggest I should see a gynecologist, primary care clinician, or GI clinician? Should I track bowel symptoms for more cycles? What symptoms mean I should call right away? Should rectal bleeding, black stool, or blood in urine change the plan? Could endometriosis, IBS, constipation, or another issue be part of the checkup? What notes would help you most at my next visit?

For bowel and bladder symptoms together, use the endometriosis bowel and bladder symptom log. For pressure patterns, use the fibroid pressure bladder bowel log.

If rectal pain or pressure is the main issue, use the rectal pain during period notes instead of turning this sheet into a full diary.
