A plain bowel and bladder symptom log for people tracking painful bowel movements, constipation, diarrhea, rectal pain, urinary urgency, painful urination, bleeding, and cycle timing.

Bowel and bladder symptoms can feel hard to say out loud. They can also be easy to leave out of a short visit.

Use this log if symptoms change near your period or ovulation. It is for notes only. It cannot tell you if you have bowel endometriosis, bladder endometriosis, IBS, a UTI, or any other condition.

Write what happened. Do not force a medical word.

"Sharp rectal pain before period" is useful. So is "urgent pee every hour on day 1."

Bladder symptoms to check

I need to pee often. I need to pee right away. Peeing hurts. My bladder hurts when full. My bladder hurts after peeing. Symptoms get worse near my period. I have blood in urine. I was checked for a UTI. UTI care did not explain the pattern.

Do not assume bladder symptoms are endometriosis. Infection and other causes need care too.
