Questions for the clinic

Pick the questions that fit.

Was a biopsy done? Were samples sent to pathology? Where will results appear? Who will explain the results? What instructions should I follow now? What symptoms should make me call? Which number should I use after hours? Should I keep tracking symptoms? Should I bring notes to a follow up visit? What privacy safe record should I keep?

Ask your clinic for rules about sex, tampons, bathing, medicine, work, school, exercise, and activity. Do not use this sheet for those rules.

Recovery notes can include clinic names, HPV, biopsy, bleeding, sex questions, medicine, location, insurance, and portal screenshots.

Before you save or share, ask:

Does my clinic need this detail? Is a short note enough? Does this include screenshots? Does this include another person's name? Will this be backed up to cloud storage? Will this go in a portal? Who can see the portal?

FTC guidance says health information can include more than diagnoses and treatment. Small details can still imply health facts.
