A period visit can feel rushed.
You may need to explain pain, bleeding, mood, sex, pregnancy worries, birth control, labs, or medicine. That is a lot to hold in your head.
Use this template before, during, and after the visit. Keep it short. Share only what helps the visit.

If the visit is short, start with this page.
Before the visit
Do this the day before, if you can.
Pick one main concern. Write the date it started. Write your last period date. List current medicines. List vitamins and supplements. Add allergies, if any. Add recent tests, if any. Add one question you need answered.
Rough notes are fine. A clear list helps more than a perfect story.

Use one row for each change.

Pick the ones that fit.
What pattern do you see in my notes? What symptoms should I track next? What symptoms mean I should call sooner? Do I need a lab test? Do I need imaging? Do I need a follow up visit? Should I bring pharmacy questions to a pharmacist? Which notes should I keep private? What will show in the patient portal? Can I get a copy of my visit summary?

Personal record notes

For this visit, that may mean:
Date of the visit. Clinic name. Main concern. Tests ordered. Medicine names to review. Follow up date. Questions for next time.
Do not put passwords, claim numbers, full receipts, or portal screenshots in a period app.

Medicine details can matter in a period visit.
Use the medication and cycle notes list to list medicines, vitamins, supplements, timing, side effects, and pharmacy questions.
Do not start, stop, or change medicine from a worksheet. Ask a clinician or pharmacist.
Short visit script
Use this if you freeze up.

HIPAA gives people some rights with covered health records. It is not a promise that every health detail stays private everywhere.
