Questions for the clinician

Pick the questions that fit.

What is the exact result? Can you say it in plain words? Which part of the report matters most? Is any result still pending? Do I need a follow up visit? When should I expect the next message? Who should I call with questions? What should I write down for my own record? Should I bring this report to another clinician? What will show in the portal? Can I get a copy of the full report? Can I ask for a correction if a detail is wrong?

This list does not say what care you need. It helps you ask your own care team.

Private record check

Biopsy result notes can include HPV, sex, pregnancy context, clinic names, lab names, insurance, and follow up visits.

Before saving or sharing, ask:

Do I need the full report here? Is a short note enough? Does this include portal screenshots? Does this include insurance details? Does this include another person's name? Will this be backed up to cloud storage? Who can open this portal account?

HHS says HIPAA gives people rights over many covered health records. It does not mean every health note in every app is covered.
