Colposcopy Question Checklist
A plain question checklist for a colposcopy visit, with prompts about why it was recommended, what records to bring, words to ask about, and privacy questions.
Use this before a colposcopy visit.
It helps you ask clear questions. It does not tell you how to prepare for the visit. Follow the clinic's own instructions.
NCI says colposcopy may be used after some abnormal HPV or Pap results. A biopsy may be discussed or done in some visits. Your clinician can explain why the visit was recommended for you.

Words to ask about
Ask for plain words.

Privacy questions
Cervical screening notes can include sex, HPV, pregnancy, period dates, clinic names, labs, portals, insurance, and follow up visits.
Ask:
Do you need my full period app history? Is a one page note enough? Will my notes go in my chart? Will results appear in the portal? Can I choose who gets portal access? Can I get a copy of my result? Can I ask for a correction if a record is wrong? Who can I call with privacy questions?
HHS says HIPAA gives people rights over many covered health records. HIPAA does not cover every place health data can live.

Short visit script
Use this if the room feels rushed.
"I am here because my result led to this visit. I brought the wording I have. Can you tell me why colposcopy was recommended and what questions I should ask today?"
Then hand over your note.
After the visit
Write down what you heard.
Question Your note What did the clinician say today? Was a biopsy discussed? Was a biopsy done? What result am I waiting for? Where will the result appear? Who should I call? What privacy question remains?
