Portal and record check

HHS says many people have rights to see and get copies of health records from covered providers and health plans.

Ask your clinic:

Where is the full lab report? Can I download a copy? Can I get older Pap or HPV results? Can I correct a wrong date or name? Who can see this portal result? Does proxy access affect this result?
