Pad Rash Symptom Log
A no-diagnosis worksheet for pad rash notes, skin timing, period day, product timing, photo privacy, care questions, and a one-page visit summary.
Pad rash can be hard to explain later.
The skin may look different by the visit.
This log keeps the story in order.
It does not say a pad caused it.
It does not diagnose the rash.
It does not tell you what product to use.
For a wider product record, use the period product irritation log.
For more skin detail, use the vulvar rash skin change notes.
This page is educational. It is not medical advice.
First rash note
Fill this in when you notice the rash.
Question Your note Date and time Period day, if known Where you see or feel it What changed redness / bumps / raw skin / swelling / cracks / sores / other Itching, 0 to 10 Burning, 0 to 10 Pain or soreness, 0 to 10 Skin feels wet or raw yes / no / not sure Discharge or smell changed yes / no / not sure Pad or liner used before this Other product used before this Main care question
Use plain words.
"Red patch near pad edge" is enough.
Do not try to name the cause.
Daily skin and rash note
Use one row per day.
Date Period day Location Look Feel Itch 0 to 10 Burn 0 to 10 Better, worse, or same Note : : labia / crease / thigh / butt / not sure red / dark / bumpy / smooth / cracked / sore itchy / raw / burning / stinging / sore better / worse / same
This table shows change over time.
It does not say what the rash is.
If burning is the main issue, use the vulvar burning symptom notes.
Product and timing note
List items that touched the area or nearby fabric.
Date Time Item New or usual Where it touched Rash timing Note pad / liner / wipe / soap / detergent / underwear / medicine / other new / usual / not sure vulva / underwear / thigh / not sure before / during / after
This is a timing note.
It is not a product safety list.
It is not a product rule.
You can use it in a care visit if it helps sort the pattern.
For more product detail, use the vulvar irritation product exposure log.
Period context
Skip any row that does not fit.
Question Your note Bleeding day light / medium / heavy / spotting / none Pad or liner change times Sweat or heat today yes / no / not sure Long walk, workout, or travel yes / no / not sure Tight clothing today yes / no / not sure Sitting for a long time yes / no / not sure Dryness before bleeding yes / no / not sure
These notes can help with the visit story.
They do not prove a cause.
If dryness is part of the pattern, use the vaginal dryness before period notes.
Photo privacy
Photos are optional.
Text notes may be enough.
If you take a photo, decide where it will live.
Photo question Your plan Will I take a photo? yes / no / not sure Where will it be stored? Who can see that place? Will it sync to another device? yes / no / not sure What will I share at care? What will I delete later?
A photo is only a record.
Screenshots and exports can travel.
Some details may help care; others may be extra.
Read the period product symptom data privacy checklist before you share.
Care questions
Pick the questions that fit.
Do my notes suggest I need an exam? Do I need a lab test? Could this be dermatitis? Could infection need testing? Does the rash location matter? Does period timing matter? Could a product, fabric, sweat, or friction matter? Should I bring photos, written notes, or both? What symptoms mean I should call sooner?
For visit prep, read Floriva for gynecologist prep.
One page summary
Fill this in before a visit or message.
Summary item Your note Main skin change Start date Period day when it started Location Worst itch score Worst burn score Raw skin, swelling, sores, or cracks Product timing Photo plan, if any Main care question
Start with what changed first.
You can add what changed next if it helps.
Privacy note
Pad rash notes can include photos, products, symptoms, and period timing.
Consider keeping the shortest useful note.
Floriva can keep short notes on your device.
Paper works too.
On device notes do not cover screenshots.
They do not cover exports, backups, or shared phones.
