I forgot a task. I forgot a step in a task. I lost an item. I missed a message. I missed a meeting, class, or call cue. I forgot a word or name. I forgot why I entered a room. I opened an app or tab and lost the reason. I repeated the same check. I forgot a care task. I forgot an errand. I forgot something with money or bills. I caught the slip myself. Someone else caught it. It affected something important.

After bleeding starts, answer what you can.
Question Your note How many memory slips did I log? How many days before bleeding did they start? Did they ease after bleeding started? Which type happened most? Did poor sleep happen too? Did pain, headache, or mood changes happen too? Did slips happen outside period timing too? What should I ask a clinician?
