Period brain fog notes for focus changes, cycle timing, task impact, symptom context, private summaries, and visit questions.
Period brain fog can be small or loud.
You may forget a word. You may reread one text too many times. You may feel fine later and wonder what happened.
Use these notes to catch the pattern. Write down what felt foggy, when it happened, and what else was going on. Do not use the notes to name the cause.
If brain fog is new, severe, or feels very different for you, you can discuss it with a clinician.

0 = none. 1 = mild. 2 = changed my day. 3 = hard to manage.

ACOG says PMS may include problems concentrating. The Office on Women's Health says PMDD can include trouble thinking or focusing. Your log can show when brain fog happened. It cannot tell you why.
Focus and memory checklist
Check any line that fits.
I had to reread texts, emails, or forms. I forgot a task I meant to do. I lost my train of thought. I had trouble finding words. I made small mistakes I had to fix. I needed extra reminders. I felt slower than usual. I had trouble making a plan. I changed a task because focus felt hard. I needed help with a care task. I felt better after bleeding started. I felt worse after bleeding started. This felt new or different.
Leave blank what does not fit. The point is a clear record, not a perfect one.
