Questions for the clinic

Pick the questions that fit.

Can you explain the exact result words? Where can I find the full report? Is this note a summary or the full report? Who can answer questions about the report? Was this ultrasound linked to a mammogram? Do you need older mammogram images? Do you need older ultrasound images? How do I send old images if you need them? Which words should I copy for my own record? Can I get a copy of the images? Can I get a copy of the report? Can I ask for a correction if a detail is wrong?

Ask your clinician or imaging center to explain any report words that are unclear.

Records and access

HHS says many people have rights to see and get copies of health records from covered providers and health plans.

Ask:

Can I download the full report? Can I get the images too? Can another clinic receive the images? Who can see this portal result? Does proxy access affect this result? Can I correct a wrong name, date, or phone number?
