Use words you would say out loud. "Left outer side, pea sized spot" is enough.

Do not force a reason into the notes. Dates and words are enough.

Prior notes to gather

Check what you have. Bring only what helps.

Old period dates. Old breast pain or tenderness notes. A prior message to a clinic. A prior visit summary. A prior imaging or lab result. A medicine or hormone start date. A photo note, if you already took one.

Questions for a clinician

Pick the questions that fit.

What words should I use to describe this change? Does the side or location change what you want to check? Does the cycle timing matter for this note? What prior notes should I bring? Should I keep tracking this by cycle day? What should I write down if nipple fluid happens? What should I write down if the skin changes? What visit, exam, or test questions fit my case?
