Use this tracker for plain notes. It does not diagnose PMS, pregnancy, breast disease, or any other cause.

Do not force a reason into the notes. Dates and changes are enough.
Pattern check
After bleeding starts or the tenderness fades, answer these.
Question Your note How many days before bleeding did it start? Did it change when bleeding started? Did it happen on one side or both? Did it stay in one spot? Did swelling or heaviness happen too? Did nipple changes happen? Did skin changes happen? Did a lump or thick area worry you? Did the same pattern happen last cycle? What is the main care question?
Mayo Clinic says breast pain can be cyclic or noncyclic. Cycle notes can help sort the pattern. They still do not name the cause.
What else happened before the period?
Check only what fits.
Bloating. Food cravings. Headache. Joint or muscle pain. Fatigue. Mood change. Sleep change. Cramps. Acne or skin change. New bra, workout, injury, or pressure on the chest. New medicine or supplement to discuss.
Mayo Clinic and the Office on Women's Health list tender breasts among PMS symptoms. That does not mean PMS is the cause for you.

Could the timing fit a cycle pattern? What details should I track next? Does one sided pain change what you want to check? Does pain in one spot change what you want to check? Should I track nipple or skin changes? Should I track breast changes for another cycle? What symptoms mean I should call sooner? What should I bring to a breast exam or gynecology visit?
If a change worries you, contact a clinician. Do not wait for a perfect tracker.
