Birth Control Refill Gap Planner

A plain planner for birth control refill gaps, pharmacy calls, appointment questions, package labels, low-detail reminders, and one-page summaries.

Refill gaps are easier to handle when you spot them early.

This planner helps you list facts. It helps you ask the pharmacy or clinic what comes next.

It does not tell you how to take pills. It does not tell you what to do if you miss pills. It does not explain refill law.

Pack and refill facts

Fill this in from the package, pharmacy app, or prescription label.

Item Your note Pill name or brand Pill type, if known combination / progestin only / not sure Current pack start date Packs left Pharmacy name Pharmacy phone Refills shown Last fill date Next refill date shown Prescriber or clinic Main refill worry

If a field is unclear, ask the pharmacy to read it with you.

Gap finder

Use this to see where the delay may be.

Possible gap What I know Who to ask No refills shown pharmacy / clinic Refill too soon message pharmacy / insurer Appointment needed clinic Pharmacy out of stock pharmacy Insurance or payment issue pharmacy / insurer Delivery delay pharmacy Old phone or email pharmacy Shared account issue pharmacy / insurer

This table does not say who must fix it. It helps you find the next call.

Questions for the pharmacy

Ask these before you run out, if you can.

How many refills are left? What date can this be filled? Do you need anything from my clinic? Is there a delay I should know about? Is the medicine in stock? What will texts or emails say? Can alerts hide the medicine name? Can I remove old phone numbers? Can I remove family access? Who should I call if the refill cannot be filled?

Ask the pharmacy about its own system. Settings vary.

Questions for a clinician

Use these for a portal message or visit.

Do I need an appointment before the next refill? Do you need my pill name or pharmacy label? What records should I bring? What should I do if the pharmacy says there are no refills? Who handles refill requests in this office? How long do refill requests usually take here? What symptoms or concerns should I mention now? What should I avoid changing on my own?

Do not wait for this planner to tell you what to do. Ask your care team.

Short call script

Low detail refill reminders

Refill alerts can show private text.

Detailed label Lower detail label Birth control refill Refill note Call pharmacy about pills Call pharmacy Prescription problem Health task Clinic refill appointment Clinic note Pill pickup Pickup note

Pick a label that still makes sense to you.

One page summary

Use this before a call or visit.

Summary item Your note Pill name Pharmacy Packs left Refill status Appointment status Payment or insurance issue, if any Alert privacy concern Who I called What they said Next step

Keep the summary short. A clear one page note can help more than a long story.

Privacy check

Refill notes can include medicine names, clinic names, dates, locations, insurance records, and payment details.

Before you save a note in an app, ask:

Do I need the full medicine name here? Do I need the pharmacy address? Do I need the prescriber name? Do I need a screenshot? Would a paper note be better? Could this show on a shared screen? Could this sync to a shared account?

Keep only what helps you.

Floriva can keep short cycle and symptom notes on your device. It does not promise privacy. It does not replace your pharmacy, pill package, or clinician.
