Questions for the pharmacy
What pill type is this? Which pills are active? Does this pack have placebo pills? Where are missed pill instructions in the package? Who should I call if the package is unclear? Could a new medicine or supplement matter? What should I write down for refills? Can you print the package instructions for me?
Bring the pack or a photo of the label if you can.
Questions for a clinician
Does this bleeding pattern need a visit? Could this symptom pattern matter? What should I track next pack? What symptoms mean I should call the same day? Should I bring my pill pack or pharmacy label? Should I bring a one page summary instead of a full log? What should I avoid changing on my own?
Do not change pill timing, dose, or method from this worksheet.
