Birth Control Side Effect Tracker

A free tracker for mood, bleeding, headaches, nausea, acne, libido, breast tenderness, cramps, and care questions after starting or switching birth control.

New symptoms can be hard to sort out.

Birth control may be one part of the story. Stress, sleep, illness, other medicine, and normal cycle changes can matter too.

This tracker helps you write down timing. It does not prove cause. It does not tell you to stop, start, or switch a method.

If a symptom feels severe, sudden, or unsafe, contact a clinician.

Method start or switch notes

Fill this in once.

Question Your answer Birth control method Brand or type, if known Start date or switch date Why you started or switched Last period or bleed start date Usual symptoms before this method Current medicines or supplements Main symptoms you want to watch Main question for care

Daily symptom log

Use a 0 to 3 score.

0 = none. 1 = mild. 2 = changed my day. 3 = hard to manage.

Date Bleeding Mood Headache Nausea Acne Libido Breast tenderness Cramps Other notes : : : : : : : none, spotting, bleeding

Keep notes short.

Examples:

"Headache after poor sleep." "Nausea after morning pill." "Cramps with spotting." "Mood felt low before bleeding." "Acne flare started this week." "Less interest in sex this week."

These notes show timing. They do not prove the method caused the symptom.

Weekly pattern check

At the end of each week, circle or write the main pattern.

Week dates Main symptom Worst score Bleeding days What else was happening? Question to ask : : sleep, stress, illness, medicine, travel

Useful pattern words:

"Started after method switch." "Worse before bleeding." "Worse after missed sleep." "Only happened once." "Happened most days." "Bothered me enough to ask for help."

Mood notes

Mood can be personal. Keep this section simple.

Date Mood score 0 to 3 Main word Safety note, if needed : calm, sad, angry, anxious, numb, okay

If you feel unsafe or may hurt yourself, seek urgent help now. Do not wait to finish the tracker.

Bleeding notes

Use this if bleeding is the main issue.

Date Type Flow Pain 0 to 10 Pregnancy test note, if you choose Notes : spotting, bleeding, period like light, medium, heavy

For more detail, use the breakthrough bleeding log or the abnormal bleeding diary.

Care questions

Pick the questions that fit.

Which symptoms on my log matter most? Could this pattern be from my method? Could another medicine or supplement matter? Should I track for more time? Should I take a pregnancy test? What symptoms mean I should call right away? What are my options if this keeps bothering me? What should I avoid changing on my own?

Bring the log. You do not need perfect words. Dates and plain notes are enough.

One page summary

Fill this out before a visit or message.

Summary item Your note Method and start or switch date Main symptom Date it started Worst day Worst score, 0 to 3 Bleeding pattern, if any Other changes that week What helped, if anything What made it worse, if anything Main question

Privacy note

Side effect notes can include mood, sex, pregnancy tests, and medicines.

Track only what helps you. Leave out details you do not need.

Floriva can keep symptom notes on your device. Paper is also fine.
