Sleep and Period Symptom Summary

A practical sleep and period summary sheet for bedtime, wake time, wake-ups, period timing, PMS symptoms, fatigue, and visit prep.

Sleep notes are easier to use when they sit next to period notes.

This summary helps you see the basics: when you slept, how often you woke, where you were in your cycle, and how you felt the next day. It is not a diagnosis tool.

Do not turn one row into a rule. Look for repeated timing.

One week summary

Use this after seven days or at the end of a cycle.

Summary question Your note Which nights had the worst sleep? Did poor sleep cluster before bleeding? Did poor sleep cluster during bleeding? Which symptom showed up with poor sleep most often? How did fatigue affect the next day? What do you want to ask at a visit?
