Ovulation Symptoms Tracker
A simple tracker for mid-cycle pain, mucus, spotting, libido, LH tests, BBT shift, and notes without treating symptoms alone as proof of ovulation.
Use this tracker when you want to understand mid cycle patterns.
Do not use symptoms alone to prove ovulation. Symptoms are clues. A chart is stronger when you track more than one signal.
1. Start with your cycle dates

If your cycles are irregular, do not force the pattern into a 28 day calendar.
2. Daily ovulation symptom tracker
Use one row each day near the middle of your cycle.

3. Pain notes
Question Your notes Which side hurt, if any? How long did it last? Was it mild, moderate, or severe? Did it stop normal activity? Did it happen with bleeding? Did it happen again next cycle?
Mild mid cycle pain can happen for some people. Ask a clinician about severe pain, new pain, pain with fever, or pain that stops normal activity.
4. Mucus notes
Use simple words.

5. LH and BBT notes
LH tests and BBT answer different questions.

If you see more than one LH surge, mark each one. Some cycles have unclear signals.
6. Pattern review
After two or three cycles, ask:
Did pain happen near the same cycle day? Did stretchy or watery mucus appear before an LH positive? Did spotting happen at the same time each cycle? Did BBT rise after the suspected fertile window? Did illness, travel, or poor sleep affect the chart? Did anything feel severe, new, or worrying?
