Use one row for each test. If you test twice in one day, use two rows.

Result words
Word What to write Negative The test line is lighter than the control line. Close The test line is darker than usual but not positive. Positive The test line meets your kit rule for positive. Invalid The control line did not work or the kit says invalid.
Follow your strip brand. Some strips use line darkness. Some digital tests use words or icons.
Add the context
These notes can make the chart easier to read later.
Time of day. Fluid intake before the test. Brand or strip type. Test photo saved or not saved. Cervical mucus that day. BBT for the next few mornings. Stress, illness, travel, or poor sleep.
If you use Floriva, keep the notes short and consistent. The goal is a chart you can read later, not a long diary.
Multiple surge notes
Some people see more than one LH rise in a cycle. A strip can look close or positive, then fade, then rise again later.
Log each rise. Do not pick the first one as proof of ovulation.

Bring this record to a clinician if repeated surges, missing periods, very long cycles, or other symptoms worry you.
What not to assume
A positive LH strip does not prove an egg was released.
A negative strip does not prove ovulation will not happen later.
Symptoms alone do not prove ovulation.
For more context, compare your strip with signs of ovulation, cervical mucus throughout the cycle, and discharge before a period.

Privacy note
LH notes can show when you expected ovulation. That can be private health data, especially if you are sharing records with care.
