Use this tracker for plain notes. It does not name the cause. It does not tell you what care to choose.

Use these prompts if you feel very sick or unsure.

Am I vomiting or unable to keep fluids down? Do I feel faint or very dizzy? Is belly or pelvic pain severe? Is pain getting worse? Do I have fever? Did I pass out? Does this feel new or unsafe for me? Do I need same day care?

Use one row per day. Track before, during, and after bleeding.

Do not force a cause into the log. Write what happened.

These notes add context. They do not diagnose PMS, cramps, infection, or another cause.

After one cycle, answer these.
