Food notes can get too big, too fast.
This diary keeps the task simple. Write what you ate in broad words. Add cycle timing, cramps, bloating, bowel changes, and symptom timing. Do not use it to blame one food or start a strict diet.

Use plain words. "Bloating after dinner on period day 1" is useful. "No clear pattern" is useful too.

Use one row per day. Keep the meal notes broad.

Do not write a perfect menu. A few clear words are better than a diary you stop using.

Use this when one symptom needs more space.

Symptoms can have many causes. Food timing is one clue. Cycle timing, sleep, stress, medicine, illness, and bowel conditions may matter too.
Bowel change check
Fill this in if bowel symptoms are part of the pattern.

NIDDK describes IBS as belly pain with bowel changes. This diary cannot tell if you have IBS. It can help you ask better questions.
What not to do with this diary
Do not use this page to:
Cut out whole food groups on your own. Start an elimination diet by yourself. Start a new product to test a theory. Treat one bad day as proof. Hide severe, new, bloody, or unsafe symptoms.
If eating notes bring guilt, fear, or binge urges, stop using the food section and ask for support.
Pattern review
Answer these after one week or one cycle.

Did symptoms happen after meals, later, or the next day? Did sleep, stress, medicine, or illness change too? What do you want help with first?
A repeat pattern is worth bringing to care. It is not proof.
Questions for care
Pick the questions that fit.
Could my symptoms be tied to cycle timing? Could a bowel condition fit these notes? What symptoms mean I should call sooner? Should I track stool type next cycle? Should I bring a food diary to a dietitian? What should I avoid changing on my own? Is an elimination diet safe for me? Do you want a short summary or the full diary?
MedlinePlus suggests writing down symptoms and questions before a visit. Bring the parts that help. Leave out private details that do not.
Short visit script

Privacy note
Food and bowel notes can show health issues, routines, alcohol, work days, sex, travel, and period timing.
Before you share, ask:
Does this person need every meal note? Can I share a short summary? Can I remove names, places, or private notes? Will this go into a portal, email, or download folder? Do I want to keep a copy of what I sent?
