Birth Control Implant Bleeding Log

A practical log for tracking spotting, bleeding days, flow changes, symptoms, and clinician questions while using the birth control implant.

This guide is for education. It is not medical advice. It cannot tell you whether to keep or remove the implant.

Use the log when bleeding feels hard to explain. Bring it to a clinician if you want help with the pattern.

What implant bleeding can look like

The implant can change bleeding in many ways.

Pattern What it means to log Spotting Light marks, often not enough for a pad or tampon. Light bleeding Bleeding that needs light protection. Frequent bleeding Bleeding or spotting on many days. Prolonged bleeding Bleeding that lasts longer than your usual period. No bleeding No period or spotting for a stretch of time. Heavy bleeding Bleeding that is heavier than your normal pattern.

CDC guidance says spotting or light bleeding is common. Some users have no bleeding. Heavy bleeding is uncommon.

That does not mean heavy or prolonged bleeding should be ignored. If bleeding feels excessive, new, or disruptive, call your clinician.

Implant details

Write these down once.

Question Your answer Implant insertion date Implant brand, if known Arm where implant was placed Can you feel the implant now? Yes / no / not sure Date of last pregnancy test, if any Current medicines or supplements Recent missed pills, emergency contraception, or birth control switch Main reason you are tracking

If you cannot feel the implant, contact a clinician and ask what to use until placement is checked.

Daily bleeding log

Use one row per day.

Date Implant month Bleeding Flow Protection used Pain 0 to 10 Sex since last entry Notes Month 1 / 2 / 3 / later None / spotting / bleeding Light / medium / heavy Liner / pad / tampon / cup / none Yes / no Month 1 / 2 / 3 / later None / spotting / bleeding Light / medium / heavy Liner / pad / tampon / cup / none Yes / no Month 1 / 2 / 3 / later None / spotting / bleeding Light / medium / heavy Liner / pad / tampon / cup / none Yes / no Month 1 / 2 / 3 / later None / spotting / bleeding Light / medium / heavy Liner / pad / tampon / cup / none Yes / no Month 1 / 2 / 3 / later None / spotting / bleeding Light / medium / heavy Liner / pad / tampon / cup / none Yes / no

Short notes work best.

Examples:

"Only noticed when wiping." "Needed liner all day." "Bleeding stopped by night." "New cramps." "Bleeding after sex." "Worried because this is heavier than usual."

Weekly pattern check

At the end of each week, count the days.

Week dates Days with no bleeding Spotting days Bleeding days Heavy days Pain days Disruption 0 to 3

Disruption key:

0 = did not affect the day. 1 = annoying, but manageable. 2 = changed clothes, sex, work, school, sleep, or plans. 3 = hard to manage or made you seek care.

This score helps you talk about quality of life, not just bleeding amount.

Pregnancy and placement check

The implant is very effective when it is in place, but concerns still need care. Use this checklist if something feels off.

Check Yes No Notes I can feel the implant in my arm. I have pregnancy symptoms that are new for me. I had severe lower belly pain. I took a pregnancy test. I contacted a clinician.

Call a clinician if you think you may be pregnant, have severe pelvic pain, or cannot feel the implant.

When to contact a clinician

Use this list as a prompt. It is not a full medical rule.

Contact a clinician if:

Bleeding is heavy or lasts longer than usual. Bleeding keeps coming back and bothers you. You feel dizzy, weak, or short of breath with bleeding. You have new severe pelvic pain. You have bleeding after sex that is new for you. You think you may be pregnant. You cannot feel the implant. The implant feels bent or broken. You have pain, swelling, warmth, drainage, or color change at the arm site.

Seek urgent care for severe symptoms such as chest pain, sudden shortness of breath, sudden severe headache, weakness on one side, trouble speaking, or sudden vision loss.

Questions to bring to the visit

Pick the questions that match your log.

Your pattern Ask this Spotting most days "Is this an expected implant pattern, or should we check another cause?" Long bleeding stretch "What options can help with prolonged bleeding?" Heavy bleeding "Do I need testing for anemia or another cause?" No bleeding "Is no bleeding okay with my implant and health history?" New pelvic pain "Should we check for pregnancy, infection, cysts, or another cause?" Bleeding after sex "Should my cervix or vagina be checked?" Pattern is affecting life "When should we consider treatment, switching methods, or removal?"
