UTI and Urinary Symptom Tracker
A no-diagnosis UTI and urinary symptom tracker for burning, urgency, frequency, cycle timing, sex timing, recurrent episodes, and visit prep.
Burning, urgency, peeing often, and bladder pressure can show up on their own. They can also show up near sex, near your period, or over and over.
This page is one tracker for all of it. It does not diagnose a UTI, cystitis, pregnancy, or any other cause. It helps you bring clear notes and good questions to a clinician.
It is not medical advice.
Call out symptoms
Contact a clinician promptly if any of these fit.
Fever Back or side pain Nausea or vomiting Blood in urine Pregnancy or possible pregnancy Symptoms feel severe Symptoms are getting worse
If you feel very sick, seek urgent care.
Mayo Clinic says back or side pain, high fever, chills, nausea, and vomiting can happen when the kidneys are affected.
One page visit sheet
Fill this out first, before a visit or portal message.
Question Your note Main symptom Date it started Burning when peeing Strong urge to pee Peeing often, with little urine Lower belly pressure Cloudy, bloody, or strong smelling urine Fever, chills, back pain, or side pain Nausea or vomiting Pregnancy or possible pregnancy Sex timing, if you want to share it Period timing, if relevant Number of episodes like this in the past year Main question for the clinician
Use plain words. "It burns when I pee" is enough.
Daily symptom log
Use one row per day. This works for a single episode, a period timed pattern, or a recurring one.
Date Cycle day Period day Urgency How often you peed Burning 0 to 10 Pelvic pressure Fever Back or side pain Blood in urine Notes : : none, mild, strong usual, more, a lot none, mild, strong yes, no yes, no yes, no, not sure
Write what changed your day. "Had to pee every hour" helps. So does "left work early."
Context that may matter
Only share what is needed for care.
Context Your note Symptoms near sex, if you want to share Symptoms near period timing New birth control or spermicide use Perimenopause or menopause context Diabetes or blood sugar concerns Kidney stone history Trouble emptying bladder Pregnancy or possible pregnancy Recent catheter or procedure
Sex timing and period timing can be clues in the visit story. They do not mean sex or your period caused your symptoms.
Recurrent episode timeline
If this keeps coming back, write one row for each episode you remember.
Date symptoms started Main symptoms Urine test done? Culture done? Medicine used Did symptoms return? yes / no / not sure yes / no / not sure yes / no / not sure
Rough dates are okay. Office on Women's Health and Mayo Clinic say recurrent UTIs can mean two UTIs in six months or three in a year. Ask your clinician how your history fits.
Test records to gather
Bring portal screenshots or lab names if you have them.
Record Date Result, if known Question Urine analysis Urine culture Pregnancy test, if relevant STI testing, if relevant Imaging, if done Scope exam, if done
Ask what each result means. Also ask what it does not rule out.
What not to assume
Do not assume:
A period means urinary symptoms are normal. A symptom near sex or ovulation has a clear cause. A past UTI means this is the same thing. A log can replace a urine test. An app can tell you what treatment you need.
The goal is clearer notes, not self diagnosis.
Questions to ask
Pick the questions that fit your situation.
Do my symptoms need a urine test today? Do I need a urine culture? Do my records fit recurrent UTI? Could another cause fit my symptoms, such as cystitis, pregnancy, or a pelvic floor issue? Should pregnancy change what we check? What symptoms mean I should call sooner? Do you need my full history, or is this summary enough? Should I see gynecology, primary care, or urology?
Do not use this page to choose antibiotics or change medicine. Ask your clinician what care fits you.
Short visit script
"I have had urinary symptoms times since . My main symptoms are . I have test results from . I want to know what needs testing and what follow up fits."
Start with the pattern. Then show the table.
One page visit summary
Fill this in before you call or go in.
Summary item Your note Symptoms started Worst burning score Worst urgency score Peeing often Fever, back pain, nausea, or vomiting Blood in urine Pregnancy concern Number of past episodes Main question
Privacy note
Bladder notes can include sex timing, pregnancy worry, urine culture results, medicine, and location clues. Share the smallest useful record.
Ask:
Do you need my full app history? Is this summary enough? Will these notes go in my chart? Can I leave out private notes not tied to care?
Floriva can keep dates and notes on your device. Paper works too. No app can promise full privacy. You still choose what to type, export, screenshot, print, or share.
For privacy help, read the UTI and bladder data privacy checklist. For visit prep, read Floriva for gynecologist prep.
Floriva note
Floriva can keep short symptom and cycle notes on your device. It does not diagnose a UTI or any other condition. A clinician can review your pattern and decide what testing or care fits.
