A plain sleep and insomnia log for period timing, wake-ups, hot flashes, night sweats, perimenopause context, daytime impact, and visit prep.

Sleep can be hard to explain in a short visit. You may remember the worst night and forget the pattern.

This log puts sleep notes next to cycle notes. It covers trouble falling asleep, wake ups, night sweats, and hot flashes in one place. It does not tell you the cause. It does not give sleep treatment advice.

If you do not know your cycle day, use plain timing. Try "3 days before bleeding" or "period day 2."

ACOG lists insomnia and fatigue as possible PMS symptoms. The Office on Women's Health lists sleep problems, hot flashes, and night sweats as symptoms that may occur during the menopause transition. Your log can show timing. It does not name the cause.

Symptoms that showed up too

Check what fits.

Fatigue. Low mood. Anxiety. Irritability. Poor focus. Headache. Cramps or pelvic pain. Breast pain. Night sweats. Hot flashes. Food cravings. Bleeding started or stopped.

Evening context

NHLBI's sleep diary includes caffeine, alcohol, medicine notes, and daytime sleepiness. Use the same idea here.

Do not turn one row into a rule. Look for repeated timing.

Perimenopause context

Use this table if periods are also changing, or if hot flashes and night sweats feel connected to a bigger shift.

If you no longer bleed, write "no bleeding" or "postmenopause." If bleeding starts after 12 months with no period or spotting, tell a clinician.

Pattern check

Answer these after a week, after your period starts, or after a full cycle.

Question Your note How many nights were hard? How many days before bleeding did it start? Did sleep change once bleeding started? Did night sweats or hot flashes happen too? Did fatigue or mood change too? Did this happen in past cycles? Did it happen far from period timing too? What do I want to ask?

The answer can be "not sure." That still helps.

Questions for a clinician

Pick the questions that fit.

Could my cycle timing matter? Could this fit a PMS or perimenopause pattern? What other sleep causes should we check? Should I keep a sleep diary for another cycle? Which symptoms mean I should call sooner? Should I bring a sleep summary instead of all notes? What private details can I leave out?

MedlinePlus suggests bringing questions and symptom notes to a visit. Keep your list short enough to use in the room.
