Thyroid and Hormone Lab Organizer

A plain lab and symptom organizer for thyroid tests, ferritin, and hormone therapy discussions, plus a monthly log for sleep, energy, and mood by cycle phase.

Thyroid and hormone questions can spread across many visits, portal messages, and lab reports.

This page holds short organizers for common situations. Pick the one that fits. None of these sections explain lab values, name a cause, or tell you what care to choose.

If you feel very unwell, contact care. If a clinician gave you urgent steps, follow them.

Thyroid lab result organizer

Use this to keep visit facts in one place. Do not use it to read a result on your own.

Test name Result date Lab or clinic Where it is saved Question for care TSH T4 Other thyroid test

MedlinePlus says thyroid blood tests can include TSH, T3, T4, and thyroid antibodies.

Question Your note Why was the test ordered? What symptom or cycle change led to the test? Last period start date Was flow heavier, lighter, late, or missed? Was fatigue part of the visit?

Ask: What thyroid tests were ordered? Where can I see the full result? Do you need the full report or a short summary? Can you explain what these results mean for my care?

Thyroid and period change notes

Use this when a period change makes you want to ask about thyroid testing. It does not tell you the cause.

Prompt Your note Main period change When it started Last three period start dates Flow change lighter, heavier, mixed, not sure Any skipped periods Biggest daily impact Thyroid question I want to ask

Office on Women's Health says thyroid hormone changes can affect periods. Too much or too little thyroid hormone can be part of light, heavy, irregular, or stopped periods. NIDDK lists heavy or irregular periods among symptoms that can occur with hypothyroidism.

Body notes to bring

Symptom or note Yes, no, or not sure Tiredness Feeling cold or hot Weight change Hair or skin change Heart racing or slow feeling Mood or focus change Sleep change

Irregular period thyroid questions

Use this when periods are early, late, skipped, closer together, or hard to predict. This list does not name the cause.

Question Your note Last period start date Period before that Longest gap Any skipped months Any spotting between periods What changed from normal

Ask: Does my pattern count as irregular for me? Could thyroid testing fit this pattern? Which thyroid test names might come up?

Test name What I want to ask TSH T4 Other thyroid test

MedlinePlus has pages on TSH and T4 tests. Ask your clinician how any test fits your symptoms and history.

Ferritin result questions

Use this when a ferritin result appears in a portal and you want clear questions. This does not read the number or range for you.

Prompt Your note Result date Was CBC also done? yes / no / not sure Was hemoglobin shown? yes / no / not sure Heavy periods lately? yes / no / not sure Low energy, dizzy, or weak? yes / no / not sure Main question

NHLBI lists ferritin with CBC, hemoglobin, and blood iron as blood tests that may be used when checking iron deficiency anemia, and lists heavy menstrual periods as a possible cause or risk factor.

Ask: What does my ferritin result mean for me? Should it be read with my CBC, hemoglobin, or blood iron? Does my heavy period history matter here?

HRT discussion questions

Use this when you want to ask about hormone therapy. This page does not say hormone therapy is right for you and does not give dose or regimen advice.

Goal Your notes Hot flashes Night sweats Broken sleep Mood changes Vaginal dryness Bleeding changes Other goal

Ask which goals hormone therapy may help and which may need another plan.

Bleeding history to bring

Question Your notes Last period start date Usual cycle length now Longest gap between periods Any bleeding after sex Any bleeding after 12 months with no period Current birth control or hormones

ACOG says hormone therapy may help with symptoms such as hot flashes, night sweats, and vaginal dryness, and risks and benefits should be reviewed with an ob gyn. ACOG says perimenopausal bleeding changes should be discussed with an ob gyn, and bleeding after menopause should be checked.

Ask: Is hormone therapy an option for me? What risks apply to my health history? What are non hormone options I should know about? How often should we review the plan?

Do not change or stop medicine without asking your clinician.

Monthly hormone health log

Use this to track sleep, energy, mood, and symptoms by cycle phase across a few months. This can help you and a clinician see whether a pattern follows your cycle.

Cycle day Phase Sleep (1 to 5) Energy (1 to 5) Mood (1 to 5) Symptoms Notes menstrual, follicular, ovulatory, luteal

Rate sleep by how rested you feel on waking, not hours slept. Rate energy by how you feel across the day, not a peak after coffee. Rate mood by your baseline emotional tone, not your reaction to one event.

Use the notes column for anything that might explain a change, such as stress, illness, travel, or a medicine change.

Reading the pattern after a few months

Look for symptoms that show up in the same phase more than once. Those are more likely tied to your cycle. Look for symptoms that show up at random times. Those may be tied to something else. Bring a short summary, not the full daily grid, to a visit.

This log can support a conversation about PMS, PMDD, perimenopause, or a hormone therapy follow up, but it does not diagnose any of these. A clinician reviews your full history alongside any pattern you bring.

Privacy note

Thyroid and hormone notes can include period dates, fatigue, weight, mood, clinic names, and account details.

Share the smallest useful summary first. Keep the full report in the source portal and send it only if the care team asks for it.

For a deeper privacy check, read the thyroid and iron lab privacy checklist and the lab results privacy checklist.

Floriva note

Floriva can hold short notes on your device. It does not read, explain, or interpret lab reports.
