CDC explains that a Pap test checks cervical cells and an HPV test checks cells for HPV. NCI explains that abnormal HPV and Pap results need clinician review, with terms like ASC US, LSIL, HSIL, unsatisfactory, and HPV positive. ACOG explains cervical cancer screening may use a Pap test, an HPV test, or both.
Ask: What are the exact result words? Was an HPV test done too? Do you need my last Pap or HPV date? Is the full lab report in the portal?
HHS says people have rights to see and get copies of health records from covered providers and health plans.

Keep the Pap line and HPV line separate. Do not turn the words into a meaning on your own.

Ask: Which line is the Pap result? Which line is the HPV result? Was HPV typed or genotyped? Can you explain this in plain words?
Keep this note focused on lab words and questions. Leave out partner names, personal guesses about why HPV was found, and care plans.

NCI describes cervical screening result terms including ASC US, LSIL, HSIL, unsatisfactory results, HPV positive results, and colposcopy. Copy the exact term. Do not change the letters. Ask your clinician to say it in plain words.
Colposcopy question checklist
Use this before a colposcopy visit. It does not tell you how to prepare your body. Follow your clinic's own instructions.

Ask: Why was colposcopy recommended for me? Will a biopsy be discussed? If a biopsy is done, when might results appear? What will show in my portal?
Colposcopy recovery notes
Use this after a colposcopy or cervical biopsy. This is a note sheet, not a care plan. It does not give a recovery time, aftercare steps, sex timing rules, tampon rules, or activity rules. Follow the instructions from your clinic.

NCI explains that cervical biopsy samples are checked by a pathologist, and that biopsy reports may use CIN wording for abnormal cervical cells found on the cervix surface. CDC says clinicians decide how best to follow up on abnormal cervical screening results.

Breast lump cycle notes
A breast lump before a period can be hard to explain. This sheet does not tell you the cause, and it does not say the lump is from hormones or is harmless.

Mayo Clinic says cyclic breast pain is related to the menstrual cycle and may come with swelling, fullness, or lumpiness. Mayo Clinic lists breast cyst symptoms that can include a movable lump, nipple discharge, and size or tenderness changes around a period. Cleveland Clinic says a clinical breast exam may be used for breast changes such as pain, skin changes, nipple fluid, or lumps.
Use words you would say out loud. "Left outer side, pea sized spot" is enough. Do not force a reason into the notes.
Breast cyst symptom notes
This is a question list, not a diagnostic checklist. Do not use it to decide if a breast change is a cyst.

Mayo Clinic lists breast cyst symptoms that can include a movable lump, nipple discharge, pain or tenderness near the lump, and size or tenderness changes around a period. Mayo Clinic says evaluation for a breast cyst may include a breast exam, imaging, or fine needle aspiration. NCI lists breast conditions that may involve lumps, pain, nipple discharge, or symptoms around the menstrual period. Cleveland Clinic says follow up tests after an abnormal clinical breast exam may include mammogram, ultrasound, breast MRI, or biopsy.
This list does not rank danger and does not decide whether a change is a cyst, harmless, or cancer. Those are clinician questions.

Ask your clinic or imaging center:
Can I download the full report? Can I get the images too? Who can see this portal result? Does proxy access affect this result? Can I correct a wrong name, date, or phone number?
Privacy note
Cervical and breast result notes can include sex history, HPV status, images, clinic names, insurance, family history, and portal messages.
Keep the smallest useful note.
