PCOS Acne and Hair Growth Photo Log

A monthly PCOS acne tracker for photos, symptom notes, lighting, location, severity, scalp hair changes, and clinician questions.

Use this photo log when acne, hair growth, or scalp hair changes are part of your PCOS care plan. It is for notes only. It does not diagnose PCOS. It does not estimate androgen levels.

Photos can help when you take them the same way each time. A blurry photo in one room and a bright photo in another room can make a change look bigger or smaller than it is.

Photo setup

Use the same setup each month.

Use the same severity scale each time.

Score Plain meaning : 0 Clear for this area. 1 A few small spots or hairs. 2 Mild change you notice. 3 Clear change you want to discuss. 4 Painful, spreading, or hard to hide. 5 Severe or very upsetting.

The score is your note. It is not a lab value.

What to write under each photo

Short notes work best.

Date. Cycle day, if known. Area shown. Lighting and room. Acne score. New or thicker hair growth. Scalp shedding or part line change. New medicine, supplement, skin product, or hair removal. Stress, sleep loss, illness, or travel if it mattered.

If you do not know the cycle day, write "not sure." Do not guess.

What not to claim from photos

Photos can show visible change. They cannot tell you why the change happened.

Do not use photos to decide:

Whether your androgen level is high. Whether you have PCOS. Whether a medicine is safe or right for you. Whether to start or stop a treatment.

Ask your clinician what the photos mean with your history, cycle notes, exam, and labs.
