Luteal Phase Date Notes
A plain luteal phase date notes worksheet for cycle day ranges, symptom timing, mood notes, privacy choices, and visit questions.
Late cycle notes can get messy.
You may feel a symptom and wonder where it fits. You may also forget the date later.
Use this sheet to mark dates and questions. Do not use it to confirm ovulation, plan pregnancy, prevent pregnancy, or name a health issue.
Set the cycle dates
Start with what you know.
Field Your note Period start date Cycle day 1 date Today's date Cycle day today Next period start date, if it happened Full cycle length, if known
If the next period has not started, leave that line blank.
Do not force a date to fit a phase.
Late cycle date notes
Use this for the second half of a cycle, if that is the part you want to watch.
Date Cycle day Bleeding or spotting Mood Sleep Body note Life note Question : 15 none / spotting / bleeding 16 none / spotting / bleeding 17 none / spotting / bleeding 18 none / spotting / bleeding 19 none / spotting / bleeding 20 none / spotting / bleeding 21 none / spotting / bleeding 22 none / spotting / bleeding 23 none / spotting / bleeding 24 none / spotting / bleeding 25 none / spotting / bleeding 26 none / spotting / bleeding 27 none / spotting / bleeding 28 none / spotting / bleeding 29 none / spotting / bleeding 30 none / spotting / bleeding 31 none / spotting / bleeding 32 none / spotting / bleeding 33 none / spotting / bleeding 34 none / spotting / bleeding 35 none / spotting / bleeding
If your cycle is longer, keep going.
Late cycle does not mean the same dates for every person.
Symptom timing box
Use plain words.
Symptom or note First date noticed Cycle day Last date noticed How much it affected life : Mood change none / a little / a lot Sleep change none / a little / a lot Headache none / a little / a lot Bloating none / a little / a lot Breast or chest soreness none / a little / a lot Cramps none / a little / a lot Appetite change none / a little / a lot Focus or memory note none / a little / a lot Other none / a little / a lot
This table does not explain why a symptom happened.
It gives you a cleaner way to tell the story.
For a broader phase guide, read menstrual cycle phases and symptoms.
For phase length background, read the luteal phase length guide.
Date compare sheet
Use this after two or more cycles.
Cycle Period start date Day symptoms began Cycle day symptoms began Next period start date Note : 1 2 3 4
Ask:
Did the same symptom show up near the same cycle day? Did sleep, stress, illness, or travel line up with a change? Did the note happen before bleeding, during bleeding, or after bleeding? Do I want to ask a clinician about this?
Do not turn one cycle into a rule.
Questions for a visit
Bring questions, not a label.
Prompt Your note I want to ask about this late cycle symptom It affected my sleep, school, work, care tasks, or plans by It happened on these dates I am unsure what to track next because I want to know which details are useful to share
If your cycle is hard to count, use the cycle day counting chart.
If timing changes a lot, read how to track an irregular menstrual cycle.
Privacy check
Late cycle notes can include mood, pain, sex, medicine, work, school, and private health questions.
Before you share them, ask:
Can I share date ranges instead of full notes? Can I remove sex or location details? Can I remove names? Can I keep private worries off the export? Will this copy stay in email or a portal? Can someone else see my device?
The FTC tells health app makers to minimize data. You can use the same rule for your own notes.
Soft Floriva note
Floriva can keep short cycle and symptom notes on your device.
That can help if you want fewer company held copies.
Floriva does not confirm ovulation, plan pregnancy, prevent pregnancy, name health issues, tell you what care to use, or make exact phase promises.
For a full date sheet, use the menstrual cycle calendar template.
