Thyroid Period Change Notes

A plain worksheet for period changes that may need a thyroid question, with a timeline, symptom notes, lab-name prompts, and a short visit script.

Period changes can feel hard to explain.

This sheet keeps the story short. It helps you show what changed and ask if thyroid testing belongs in the visit.

It does not tell you the cause. It does not explain lab results.

One page change note

Fill this out first.

Prompt Your note Main period change When it started Last three period start dates Usual period pattern Current pattern Flow change lighter, heavier, mixed, not sure Any skipped periods Any bleeding that will not stop Biggest daily impact Thyroid question I want to ask

Use plain words.

"Two periods came close together." "Flow got much heavier." "Period stopped for two months." "I feel tired and cold too."

Period change timeline

Use one row for each cycle or month.

Month or date Period timing Flow Other body changes What changed from normal early, late, skipped, on time light, usual, heavy

Office on Women's Health says thyroid hormone changes can affect periods. Too much or too little thyroid hormone can be part of light, heavy, irregular, or stopped periods.

That fact is a reason to ask a question. It is not an answer by itself.

Body notes to bring

Check what fits. Leave the rest blank.

Symptom or note Yes, no, or not sure Details Tiredness Feeling cold or hot Weight change Hair or skin change Neck swelling or pressure Heart racing or slow feeling Constipation or diarrhea Mood or focus change Sleep change Follow up question I want to ask

Do not use this table to rank symptoms. Use it to remember what to say.

Lab name list

If you already have lab names, write them here.

Test name Date Where it came from Question for visit TSH T4 Other thyroid test Blood count or iron test

MedlinePlus describes TSH and T4 as thyroid related tests. Ask your clinician what any result means for you.

Do not copy a full portal screenshot if a short list is enough.

Questions to ask

Pick the ones that fit.

Could thyroid testing fit my period change? Which period details matter most? Do my other symptoms matter for this question? Which tests are you ordering? What will each test help check? When should I expect results? How will I get the results? Who explains the results to me? What symptoms mean I should call sooner? What should I track before follow up?

For heavy bleeding, use the thyroid heavy period visit summary. For skipped or uneven timing, use the thyroid irregular period question list.

Short visit script

If care feels urgent, ask for care with the notes you have.

Privacy note

Thyroid and period notes can include lab names, symptom context, clinic names, and portal screenshots.

Keep the note small when small works.

Ask before you upload a full app export. A one page summary may be enough.

Read the lab results period data privacy checklist before saving or sending lab screenshots.
