Stress and Missed Period Notes
A plain notes worksheet for stress timing, sleep, meals, illness, cycle dates, late or missed period timing, and care questions.
Use this worksheet when stress and a missed period are both on your mind.
It helps you compare dates.
It cannot prove stress is the reason for the change.
It cannot tell you if you are pregnant.
It cannot rule out another cause.
Stress can be one clue to discuss with care.
Start with the cycle dates
Fill this in first.
Question Your note Last full period start date Expected period date Date you noticed it was late or missed Any spotting date Any full flow date Usual cycle length, if known Pregnancy test date, if relevant Main care question
Use plain words.
"Late by four days" is enough.
"No flow yet, high stress last week" is enough too.
Stress timing notes
Use one row per day or event.
Date Stress level What was happening? Sleep Meals Illness Period note low / medium / high good / okay / poor normal / skipped / changed no / yes
Keep details small.
"Work deadline" may be enough.
"Family stress" may be enough.
Do not add names unless they help your care question.
Late or missed period log
Use one row per day.
Date Cycle day Bleeding or spotting Pain 0 to 10 Mood or body note Test context if relevant : : none / spotting / flow
This is a timing record.
It does not name the cause.
NHS lists many contexts for late or missed periods.
Stress is one context on that list.
Sleep, meals, and illness
These can help you explain the week.
Date range Sleep change Meal or appetite change Illness Exercise change Travel Notes
Do not turn one row into an answer.
Use it to ask better questions.
MedlinePlus lists emotional distress, exercise, low body fat, sudden weight loss, medicines, and health issues among contexts for absent periods.
Pregnancy test context, if relevant
Skip this if testing is not part of your question.
Test detail Your note Test date Days before or after expected period Result you saw negative / positive / unclear What the test label said Follow up question
This sheet does not explain a result.
It does not tell you when to test.
Office on Women's Health says timing can affect home test accuracy.

Questions for care
Pick the ones that fit.
Could stress be one context to discuss? What else should we check? Does my timing need follow up? Should pregnancy testing be part of this question? Could sleep, meals, illness, travel, or exercise matter? Could medicine or birth control changes matter? What symptoms should make me ask sooner? How many cycles should I track? Should I bring a summary or the full log?
MedlinePlus says it can help to write down symptoms and questions before a visit.
