Use this 2-cycle PMDD tracker to log daily mood, body symptoms, function, period start, and recovery for a clinician visit.
Track daily, not from memory
PMDD is about timing and impact. Symptoms usually rise before a period and ease after bleeding starts. That pattern is hard to prove from memory.
Use this tracker once a day for 2 full cycles. Keep going for a few days after each period starts. Those recovery days matter.

Use the same time each day if you can. Night is often easiest.
Rating scale
Use 0 to 4 for each symptom.
0: not present 1: mild 2: clear, but manageable 3: hard to function 4: severe or unsafe
For function, use the same scale:
0: normal day 1: slower than usual 2: some tasks dropped 3: work, school, care, or relationships were disrupted 4: could not function safely
What to include in body symptoms
Use the body symptoms column for:
Breast tenderness Bloating Headache Cramps Fatigue Sleep changes Food cravings
You do not need perfect detail every day. Daily scores matter more than long notes.

After 2 cycles, fill this out.

How a clinician may read the pattern
ACOG describes PMS and PMDD as symptoms that happen before a period and improve after the period starts. PMDD is more severe and can disrupt daily life.
The log helps answer 3 questions:
Do symptoms cluster before the period? Do they improve after bleeding starts? Do they disrupt work, school, self care, or relationships?
If symptoms stay high all month, the answer may not be PMDD alone. It may be another mood or health condition that gets worse before a period. That still deserves care. The tracker helps show the full pattern.
Privacy note
PMDD notes can include mental health details, sex, medication use, conflict, and safety concerns. Store them with care.
