Use plain words. "Racing heart in the car" helps. "Felt like I might die" helps if it is true.
Body sensations
Check what fit. Add notes only if they help.
Racing or pounding heart. Short breath. Chest tightness or chest pain. Sweating or chills. Shaking or trembling. Weak or dizzy. Nausea or stomach pain. Numbness or tingling. Felt unreal or detached. Fear of dying. Fear of losing control. Worry about another episode. Other:
NIMH lists many of these as possible panic attack symptoms. This checklist does not say you had a panic attack or panic disorder. It gives your clinician clearer notes.

Do not force the notes to fit period timing. Write down if it happened far from your period too.

Write what changed.
Area What happened? How much did it affect me? Short notes are enough. "Left the store" is useful. "Could not drive home alone" is useful if it happened.
Pattern review
After bleeding starts, answer what you can.
Question Your note Did panic feelings happen before bleeding? How many days before bleeding? Did they happen during bleeding too? Did they change after bleeding started? Did sleep change first? Did pain show up too? Did stress show up too? Did this happen outside period timing? What made me seek help?
ACOG and OWH list anxiety in premenstrual symptom guidance. This sheet does not claim panic feelings are PMS, PMDD, or anxiety disorder. It only records timing and impact.
Questions for a clinician
Pick what fits.
Could this timing matter? What other causes should we check? Which body symptoms need urgent care? Should I track sleep, pain, stress, and safety notes too? How much detail do you want? Do you want a short summary or the full log? What symptoms mean I should call sooner? What should I track before the next visit?
