A complete menstrual migraine log covering cycle timing, aura, medicine response, triggers, appointment prep, and perimenopause pattern changes.
Migraine attacks and period days can overlap. That does not prove one causes the other.
This log gives you one place to write down migraine timing, period timing, aura, medicine response, and possible triggers. It does not diagnose menstrual migraine or choose care for you.

Head pain came on all at once (thunderclap headache). This is the worst headache you have had. Fever or stiff neck. Confusion. Seizure. Double vision. Numbness or weakness. This started after a head injury. This headache feels new or very different. You feel unsafe waiting.

American Migraine Foundation says a headache and menstrual diary can help a doctor review menstrual migraine timing, severity, and response to treatment. The Migraine Trust says a diary for at least three months can help show a link with period days.
Before period migraine days
Use this if migraine tends to start before bleeding.

AMF says attacks that start from two days before a period through the third day of flow may fit menstrual migraine. This table shows timing only. It does not prove the cause.
Aura and warning signs
Aura can matter for care choices, and some symptoms need prompt review.

Check what happened:
Flashing lights, zigzag lines, spots, or blind spots. Blurry vision or vision loss. Tingling, numbness, or weakness. Trouble speaking, slurred speech, or trouble understanding words. Confusion. No aura signs.
Mayo Clinic says new, one eye, or changed visual symptoms, new headaches, weakness, or speech changes need prompt medical review. Do not drive during vision changes. Get urgent care now for sudden weakness, numbness, trouble speaking, confusion, seizure, or double vision.

Use this on migraine days and on a few non migraine days. The quiet days matter too.

Mayo Clinic lists possible migraine triggers, including hormonal changes, alcohol, caffeine, stress, sensory stimuli, sleep changes, exertion, weather, medicines, foods, and skipped meals. This table can show a pattern. It cannot prove that one item caused an attack. If the same factor shows up on many non migraine days too, the link may be weak.
Perimenopause pattern notes
Use this if hot flashes, night sweats, or irregular cycles are part of the picture.

Mayo Clinic says estrogen changes during menstrual periods, pregnancy, and perimenopause can trigger migraine attacks for some people. This table does not diagnose a perimenopause migraine link.

Questions to ask a clinician
Pick the ones that fit your visit.
Could my migraine pattern be tied to my cycle? What else could explain these symptoms? How many cycles should I track? Should I track aura in a special way? Could my current medicine cause problems if I use it often? What should I do if I vomit during an attack? Do my aura notes change what treatment options are safe for me? Should I see a headache clinician, gynecologist, or both? What symptoms mean I should seek urgent care?
