Long Period Bleeding Log
A simple log for periods that last longer than usual, with daily flow notes, clot notes, symptoms, and visit questions.
A long period can blur together by day 8 or day 10. It is hard to remember when the flow changed, how many products you used, and what else you felt.
Use this log during the bleed or soon after. It is for notes, not diagnosis. If the bleeding feels sudden, severe, very heavy, or very different for you, you can ask for care and bring the notes you have.

Good notes sound plain:
"Brown spotting only." "Light red flow, one liner." "Changed pad every two hours." "Quarter size clot at night." "Felt weak after shower."
Flow words to use
Word What it means here Spotting Marks on underwear or liner. Light A liner or light product is enough. Medium You change every few hours. Heavy You change often or plan around bleeding. Very heavy You soak through fast, leak, or feel worried.
Do not force your body into a neat box. Write what happened.
What changed?
Mark what fits.
This period lasted longer than usual. Bleeding stopped, then started again. Spotting went on for many days. Flow got heavier after day 7. Clots were new or larger than usual. Pain was new or worse than usual. I felt dizzy, weak, tired, or short of breath. Bleeding started after new medicine or birth control. Bleeding happened after sex. Bleeding happened after a missed period. Bleeding happened after menopause.
This list does not tell you the cause. It gives your clinician a cleaner story.
Questions for a visit
Pick the ones that fit.
Does this count as abnormal uterine bleeding? Does the length of this bleed need follow up? Do I need a blood count or iron check? Should pregnancy be ruled out today? Could my birth control, IUD, medicine, or blood thinner be part of this? Should I ask about fibroids, polyps, thyroid issues, or ovulation changes? Do I need an ultrasound or other exam? What should I track if this happens again? What symptoms mean I should ask for care sooner?
If the bleeding is still going, use period won't stop visit prep for a shorter visit summary.
Short visit script
Use this if the visit is quick.
