Illness and Late Period Notes
A plain notes worksheet for illness timing, fever, rest, sleep, appetite, medicine used, expected period date, late timing, symptoms, and care questions.
Use this worksheet if you were sick and your period is late.
It helps you write down what happened.
It does not tell you the cause.
It does not say illness made your period late.
It does not give medical advice.
If symptoms feel severe, sudden, or unsafe, ask for care.
For a wider timing log, use the early or late period log.
For a plain late period guide, read why is my period late.

This is not dosing advice.

Do not guess.
If you do not know the name, write "not sure."
For a longer medicine list, use the medication and cycle notes list.
Symptoms to mention
Check what you want to ask about.
Period is late. Period was missed. Spotting happened. Cramps happened with no flow. Fever happened. Sleep changed. Appetite changed. Weight changed. Stress was high. Exercise changed. New medicine was used. Stomach symptoms happened. Pain feels new. Bleeding feels unusual. I may need to ask about a pregnancy test.
This list is for memory.
It does not tell you what is wrong.
Office on Women's Health says home pregnancy test timing can affect results.
Ask a clinician if you are unsure what to do next.
Care questions
Pick the ones that fit.
Can sickness delay your period in my case? What else could explain this timing? Should I track another cycle? What symptoms should make me call sooner? Should pregnancy testing be part of this question? Could a medicine I used matter here? Should I ask a pharmacist about medicine labels? What notes do you need from me? What notes can I leave out? When should I follow up?
MedlinePlus says it can help to bring questions and symptom notes to a visit.
