First Period Doctor Question List

A plain question list for a first period visit, with prompts about bleeding, pain, school, supplies, privacy, and what notes to bring.

Use this list before a first period visit, a school nurse visit, or a parent call to a clinic.

You do not need perfect words. Bring short notes. Ask the questions that matter to you.

This list is not medical advice. It does not tell you what care to use.

For a simple log, use the first period tracking starter sheet.

Quick note to bring

Fill in only what you know.

Question My note First day I saw blood Last day I saw blood Flow words light, medium, heavy, or changed by day Worst pain, 0 to 10 Did I miss school or plans? Supplies I used What scared or confused me What I want kept private

If you do not know an answer, leave it blank.

Start the visit

Use one line.

You can ask for plain words.

Body and timing questions

These questions do not ask for a prediction.

What period changes are common at first? What changes should I write down? What details help you understand my period? What details do you not need? What should I track for the next few cycles? What period pattern should make me call the office? Who should I call after hours? What should I do if I feel scared by bleeding or pain?

MedlinePlus says periods may take time to become regular. ACOG says period patterns can be a health sign.

Bleeding questions

Ask about what you saw.

What flow words should I use? How should I describe clots? How should I describe leaks? What should I write down about color? What should I write down about how often I change supplies? When should bleeding be checked by a clinician? What should I do if I feel dizzy or faint? What should I bring if this happens again?

Do not try to label the cause yourself.

Pain questions

Use a 0 to 10 score if it helps.

Is this pain detail useful? What should I track about cramps? What should I track about back pain? What should I track about headaches? What pain level should make me call? What symptoms should I not ignore? What can I do at school if pain gets in the way? Who can help me make a school plan?

This page does not give treatment advice. Ask the clinician what fits your situation.

School and sports questions

Ask about daily life.

What should I do if I leak at school? What should I keep in my bag? What should I ask the school nurse? What note should I keep for missed class? What should I track if I miss sports? What should I track if I cannot sleep? Can you help me explain this to school?

You can share school impact without sharing every private detail.

Supplies questions

Ask about choices in plain terms.

What supply types can I ask about? How do I know when to change a pad? What should I ask about tampons? What should I ask about period underwear? What should I ask about skin irritation? What should I do if a product hurts? What supplies should I keep at school?

Ask a clinician or trusted adult before using a product you do not understand.

Privacy questions

Privacy can matter at a visit and in an app.

Can I ask part of this visit alone? What can stay out of my chart? What must go in my chart? What might show in a portal? Who can see portal messages? Can reminders use plain words? What should I put in a period app? What should I leave out of a period app?

Rules can vary by clinic and place. Ask the office how privacy works.

For app setup, use the first period app setup privacy card.

App and tracker questions

Ask before adding old history.

What should I track first? Are dates enough for now? Should I track flow? Should I track pain? Should I track school impact? Should I skip mood notes? Should I skip diary notes? Should I bring screenshots? Is a short summary better than an export?

Use less detail when less detail works.
