Endometriosis Laparoscopy Recovery Log
A plain recovery log for endometriosis laparoscopy, with daily and weekly tables for symptoms, activity, incision notes, medicines, and follow-up questions.
Use this log after endometriosis laparoscopy. It helps you keep notes for your follow up visit.
This is not a recovery plan. Your surgeon's instructions come first. Use your discharge papers for wound care, medicine, movement, bathing, food, sex, work, driving, and when to call.
If a symptom feels urgent or worrying, seek care. Do not wait to finish this worksheet.
Before you start
Fill this out from your discharge papers or clinic portal.
Item Your notes Surgery date Surgeon or clinic Procedure name What the surgeon found What the surgeon did Incision care instructions Activity limits Medicine plan Follow up date After hours phone number When your papers say to call
Write "not sure" if you do not know. Ask at follow up.
Daily recovery log
Use one row each day. Short notes are fine.
Date Day after surgery Pain 0 to 10 Pain location Incision notes Bleeding notes Bowel and bladder notes Medicine questions Activity and rest : :
Pain score guide:
0 means no pain. 3 means pain is present but manageable. 6 means pain limits normal movement. 10 means pain feels like an emergency.
Use your own scale each day. The pattern matters more than the exact number.
Incision notes
Check only what you can see. Do not remove dressings unless your instructions say to.

Bowel and bladder log
Endometriosis can involve bowel or bladder symptoms. Surgery, anesthesia, medicines, and rest can also change these patterns.

Write down what changed from your normal pattern. Bring the pattern to your follow up visit.

This table is for questions only. Do not start, stop, or change medicine because of this worksheet.

Include prescription medicine, over the counter medicine, supplements, and nausea medicine if your team told you to use them.

Follow up questions
Choose the questions that fit your case.
What did you find during surgery? Was any tissue sent to pathology? Did the findings confirm endometriosis? Were any areas left in place? Were bowel, bladder, ovary, or tube areas involved? What symptoms should improve first? What symptoms may take more time? What symptoms should prompt a call? What activity limits still apply? When can I return to work, school, sex, exercise, driving, or lifting? Should I keep tracking pain by cycle day? Should I track my first period after surgery? What should I do if old pain returns? Do I need another visit or referral? What privacy safe records should I keep?
What to bring to the visit
Bring the one page setup sheet, the weekly check in, and the medicine question log. Bring daily pages only if the clinician asks for detail.
For symptom patterns before surgery, use the endometriosis pain diary. For long term tracking, use the period tracking for endometriosis guide and the Floriva endometriosis tracking guide.
