A plain food and symptom log for endometriosis care prep, with meals, caffeine, alcohol, pain, bowel symptoms, bloating, fatigue, cycle day, and clinician questions.
Use this log when food questions are part of your endometriosis care.
It helps you write down meals, drinks, cycle timing, pain, bowel symptoms, bloating, and fatigue. It does not prove that a food caused symptoms. It does not treat endometriosis.
Bring the pattern to a clinician or dietitian. If you have severe pain, fever, fainting, blood in stool or urine, rapid weight loss, pregnancy concerns, or symptoms that feel unsafe, seek care now.

Use this when the daily row needs more detail.

Symptoms can have many causes. Food timing is only one clue. Cycle timing, sleep, stress, medicine, illness, and bowel conditions may also matter.

You may choose to track:
Meals and snacks. Caffeine. Alcohol. Dairy, gluten, spicy food, high fiber food, or fried food if those are your own questions. New supplements. New medicines. Missed meals. Very late meals.
Do not cut out whole food groups based on one bad day. Ask for help first if eating feels stressful or unsafe.
Bowel and bloating check
Endometriosis can overlap with bowel symptoms, but bowel symptoms can also have other causes.

Call a clinician sooner for blood in stool, black stool, fever, vomiting, faintness, severe pain, or symptoms that are new for you.
Pattern review
After one cycle, answer these.
Question Your notes Which cycle days were worst? Did symptoms cluster before bleeding? Did symptoms cluster during bleeding? Did symptoms happen outside the period? Did bowel symptoms follow a cycle pattern? Did any food question repeat more than once? Did sleep, stress, medicine, or illness also change? What do you want help with first?
A repeated timing pattern is worth asking about. It is not proof.
Questions for a clinician or dietitian
Bring the questions that fit.
Could my symptoms be tied to my cycle? What symptoms need urgent care? Should we check bowel, bladder, or pelvic causes? Should I see a dietitian? Is an elimination diet safe for me? What should I avoid changing on my own? Could medicine, supplements, or birth control affect this pattern? What should I track before the next visit? Would a short summary be better than a full food diary?

A short visit script
